PHP questionaire????

Hi,

I need to make a ONE php questionaire script (if possible) with drop down list for a particular answer. Also, after each question is answered i'd like to be redirected to another page for more questions....then again redirected to another page with the correct link from all the answers that was chosen. Can you please help or redirect me to the place online to develop something like this .....as i'm only a newbie
